Proxy Access

A parent/guardian can set up and manage an account for a child/dependent. This allows users the full functionality of MyHealth@ACMC to manage the health care of a dependent. In compliance with HIPAA laws and legal privacy, proxy access is available in three categories:

Minor Proxy — How to sign up

Parents/guardians can set up a minor account allowing access to view and manage a child's account until the child turns 12. From the ages of 12 to 17, parents may still view the account, but it will cease to update, displaying only information until the child's 12th birthday. This access expires when the child turns 18.

  1. Complete the Minor Proxy Access Request Form (one per child) and return it to the receptionist at any ACMC clinic. If you are the legal guardian requesting access, please fill out the Legal Guardian Proxy Access Request Form.
  2. Our Health Information department staff can assist with advising you of any other documentation necessary to the minor child's legal situation. These items may include:
    1. Adoption papers
    2. Guardianship papers
    3. Power of attorney
    4. Ward of the state or county
    5. Other
  3. Mail COPIES of any required legal documents to ACMC Health Information Department, 101 Willmar Avenue SW, Willmar, MN 56201 .
  4. Within 5 to 7 business days you will receive a notification email from 'noreply@FollowMyHealth.com' with a link to complete the proxy account.
  5. You will be asked to enter an INVITE CODE, which is either the PATIENT'S BIRTH YEAR or the LAST FOUR DIGITS OF the patient's SOCIAL SECURITY NUMBER and then complete the final steps to establish the account.
  6. Sign into the account and start using all the convenient features of MyHealth@ACMC!

Adult Proxy — How to sign up

Through the MyHealth@ACMC patient portal, legal guardians and caregivers with power of attorney can view the online medical records of adult individuals in their care, and perform tasks such as communicating with primary health care physician or advanced practice practitioner

There are two ways to grant adult proxy access: dependent adult registration, and self-assigned adult registration.

Dependent Adult Proxy

  1. Complete the Adult or Legal Guardian Proxy Access Request Form and return it to the receptionist at any ACMC clinic, or mail it with any required documentation to ACMC Health Information Department, 101 Willmar Avenue SW, Willmar, MN 56201. Documentation necessary to the patient's legal situation may include:
    1. Power of attorney
    2. Guardianship papers
    3. Adoption papers
    4. Ward of the state or county
    5. Other

Self-Assigned Adult Proxy

  1. A patient can also grant proxy access to their own MyHealth@ACMC account from within the patient portal.
  2. Log into your account and select My Account > Preferences > Manage Account > Invite Proxy.
  3. Select and complete either 'Read Only Proxy' or 'Full Access Proxy'. Then enter the email address/contact information of the individual you would like to grant proxy access to.
  4. Select a pass code for the individual to enter when accessing your account.
  5. Provide this code to the proxy.
  6. To access a proxy record from a MyHealth@ACMC account, navigate to the user's name at the top of the screen and mouse over the drop down area. Then select the proxy patient's name to view to their account. See example below.
